This is a similar problem to what has recently been a WD45 topic.  My transmission was stuck in reverse because the shift lever (WD straight) had become disengaged from the shift rods.  I've taken the lever off and looked at everything.  There is no unusual wear to my untrained eye.  We lined everything up and put it back together.  The shift lever has about 1/4 inch vertical (up/down) movement. The pivot pin doesn't keep the lever from rotating unless the lever is pulled all the way up.  My pivot washer is just a small metal piece, should it be rubber lined?  Is the bottom of the transmission cover wore were the shift lever ball rides?  I have an extra cover that I got off of ebay.  It has the shift rods, but no lever.  I didn't think to try my lever in it.  Should I?  Any and all help is greatly appreciated.

If the lever rotates it will slide between the notches below. Sounds like it is time to replace the gear shift and rebuild the pivot pin holes with over size pins.  MACK
