Around here, everyone calls that style the "Monroe" conversion

That looks like the Trasco, from Tractor Supply Company.  It sold for $23.95 in my 1957 Tractor Supply Catalog.
 
The seat you have is a Knoedler, I have the advertising literature for that seat.
