Has anyone ever seen a bull gear tear into the pan? I assume the spacer on the axle would have to fail?? Maybe the pan got hit, a stump or somehting and it got bent into the gear?? Any input here?

might want to check the snap ring that holds the gears in place. Might be gone or came off. Had that happen one one I worked on, cut a whole all the way through the pan. Found the snap ring just hangin out on the axle.

Might be the nut for the inner bearing of the bull gear is coming loose letting the bull gear move outboard. Pop the cap off the final drive housing and take a look.
