what makes the d-14 jump out of gear and if it jumps out of 3rd and 4th is this also common or a sign of bigger problems . what would be the fix ?

  have seen the nut come loose on the pinion shaft   will cause it also  was told it can be tightened from outside

The most common cause is people grinding the gears. Over time the tooth rounder on the gear becomes a deep bevel and it will not stay in gear. If this is the case, replacement of the shifting collars at least, and worse case would be to replace both gears and collar. After splitting the tractor, you might as well fix it right.
