Does anyone know if AC made a tricycle model of the D12? If so, what years?

Not available from the factory, but I have seen CA front ends put on a D-10.

I have go agree. I do not believe the D-12 was made with a narrow front.
I am away from home so I do not have Pop's Price Book to verify
