isn't it a 2 bolt flange bearing ?
if my picture of shaft and support brackets are close the bearing bolted to upright .
should be able to come up with something

any bearing house should be able to supply or measure shaft and might find at tractor supply or other farm service store . 
 If I knew the shaft size I probably have some in my parts bin - have 400/500 bearing I got when shop closed - rather than trash them I figured some day I could use them - have fixed spindles on several mowers , built few other things with them but 2 1/2 " 4 bolt flange bearings for air handlers I have not come up with use for YET .

thanks coke. I have not pressed the old bearings off yet. I do have an hd4 loader for spare parts and I pulled the fan shaft out. seems that the bearings had been replaced recently. they are tight so I installed it into the dozer. I still want to find two bearings for spares. they don't seem to last long.

bearings are generic parts like bolts. Like COKE said, they sell the two bolt flange bearings at the local Farm Store, Bearing house, NAPA, etc....... you just have to measure the shaft size and bolt to bolt  center line dimension.
Like them all, but love the "B"s.

8 years and never had problem with those bearings - sold machine and never replaced them in full time i had it . 
if they have went out before - look to see if mounting is loose or bent causing load problems or shaft is bent - that will destroy bearings fast if vibration or flexing of the shaft .
