New rims wearing around lug holes?

I recently bought a D17 and have been fixing some major and minor things.  The old front wheel rim was kind of bent, and it broke on me while I was moving some soil around with the FEL.  I bought a new rim.  The old lug bolts threads were smashed and needed chasing. 

The front rim has about 10 hours on it.  Something doesn't quite seem right.  The bolts were coming loose, and the holes in the rim are starting to deform. 
 
What should I do?

Looks like the rim isn't up on the hub good. The studs are to hold the rim up on the center. The center of the rim carries the load. If your running a FEL it's likely flexing the rim and allowing the lugnuts to work off.

Looks like your center hole in the rim is too small. I didn't notice the flats right away but I agree its on backwards.

I think the rim is on backwards. All those nice flats on the rim should be against the hub. Replace all 5 studs as well.

Check the other side - are the holes tapered or flat. You have used tapered nuts on the flat side of wheel. Other side should be tapered which is what you want.

All the above. It looks like the rim is on backwards. The lug holes in the rim should be tapered to match the taper on the nut. The center hole is formed and the radius-ed side should be towards the hub sitting flat against the hub. The center hole should fit snug on the hub. The studs should be replaced if a nut has any slop on the threads.
