Did every D15 II come from the factory with its center 3pt. link with one end category 1 and the other end category 2? Or was it an option?

3 Point hitch was optional on both Series I and II.  Those models which came with the 3 point would have had the top link that you describe - different sized sockets on each end.

I think the D15 top link was cat 1 on both ends, I have one. And the D17 top link was cat 1 on one end and cat 2 on the other,I have two. That being said I could be wrong, and you could get the top links both ways. All so the book shows a top link for the WD45 with cat 1  on one end and cat 2 on the other.

All the factory D-15II links that I have ever seed had the #1 and 2 ends.  D-10's and 12's had #1's on both ends...
